양식에 맞춰 항목별 신청서를 작성해주세요.

   * **학원명**: 맥가이에서 사용하는 학원명을 입력해 주세요.
   * **제품명**: 이용하는 서비스 제품을 선택해 주세요.
   * **신청자명**: 신청자 본인의 이름을 입력해 주세요.
     * 신청자명과 3. 학원대표(원장)명은 **다를 수 있습니다.**
   * **연락처**: 학원 연락처, 신청자 연락처를 입력해 주세요.
   * **사업자등록번호**: 학원의 사업자 번호를 입력해 주세요.
   * **주소**: 학원의 주소를 입력해 주세요.
2. 서비스 내용을 확인해 주세요.
3. **(선택)** 고객 구분을 선택해 주세요.
4. <mark style="color:red;">**(필수)**</mark> 학원대표님 날인을 찍어주세요.
5. <mark style="color:red;">**(필수)**</mark> 개인정보 동의에 체크해 주세요.
   * 미동의 시 해당 서비스 이용이 제한돼요.

맥가이 고객지원 **이메일 주소**로 **아래 내용을 포함하여 작성된 신청서**를 <support@macgai7.com> 으로 신청 메일을 보내주시면 됩니다.
